My understanding is that 100% + payouts are only relevant over a long period of time...my question is...why would a casino have 100%+ machines? Doesn't the casino lose in the long run? Is this like a sales item in the store?

Those machines will return more than 100% only to players who know how to play them properly.
Unlike slots, there is a skill involved in playing video poker.
Most people don't play their hands properly and they can't achieve the 100%+ paybacks. That's why the casinos can still make money on them.
